Simple goals may take a few weeks or a month; more complex goals may take six months to a year, or more..."
"If it is decided that an attempt is to be made to change or modify a behavior, it must be defined. In order to define it, you and any other observer must be able to see it happening. Describing the behavior is very important. Everyone must be in agreement as to what the target behavior is and what it looks like as it occurs..."
"The consumer's response may or may not be directly affected by staff interventions. Whether or not you are able to disrupt or alter your client's behavior needs to be noted. If you have been instructed, for instance to ignore "yelling" and the behavior continues or increases in frequency then this needs to be included in your notes. If on the other hand, it becomes less intense or less frequent, then this is also very important information to be shared with others who may be involved in this person's life..."
