This kit provides sample reporting forms, as well as, policies and procedures, which can be used as training guides or adapted for you to use in your program! (Instructions included on CD.) The accompanying easy-to-understand training packet includes information about the purpose and methods of gathering and reporting incidents, both "Minor" and "Special". Because of its comprehensive approach the training element can be used in conjunction with most existing incident report management systems. Simply replace the sample forms with in-house documentation!

This read-along training can be presented in a group setting or given privately to an individual needing to enhance current knowledge and skills. Packet kits include questions for discussion, a pre and post test, and test key.

Objectives:

At the end of this unit participants will be able to:

1.Discuss why it is important to monitor and document conditions that may have or might cause harm to consumers, staff, and those who may come to visit their program.

2. Understand and apply their knowledge of what constitutes conditions for use of:

a. Minor Event Reports

b. Special Incident Reports

3. Understand and be able to fill out Minor Event Report and Special Incident Report forms.

4. Know and understand their own program's Policies and Procedures for use of SIR and MER forms. (Other designations for similar reporting forms may be substituted for these incident/accident and unusual occurrence reporting systems.)

"It is the responsibility of every person who provides care or supervision to others to determine whether existing conditions are safe and free from danger! Your observations will insure that hazardous conditions or events do not impact the welfare, safety, or health of the consumers you serve, the staff you work with, or those who may come to visit your program or facility..."

"When care and supervision programs are established, laws and regulations are written to protect those being cared for by others. Lawmakers have a duty to create such regulations. These laws do not guarantee that nothing will ever happen in such settings but they do go a long way towards providing safe and healthy conditions for those receiving these services. All licensed businesses must follow similar rules in order to get and keep their licenses. However, programs that deal with the daily care and treatment of people do tend to have stricter guidelines..."

"In most cases where a Minor Event Report form is to be used, you will certainly have to determine the urgency of the situation. Do not assume that by filling out a MER that you would not also need to speak directly to your supervisor. This is especially true regarding incidents where even slight injuries have occurred. Consumers who may have accidentally fallen may not appear to be injured but may be required by program policy to be examined to rule out the possibility of future medical problems. Consumers with moderate to severe communication disabilities may also need to be given special attention..."

"NOTE: Take some time now to look over the "sample" Special Incident Report form. Don't panic! You will only be involved in the completion of this form if and when your program administrator(s) determine it will be needed. However, you should have a good working knowledge of which conditions might result in the use of the SIR. (Details of conditions are coming up in the next section of this packet.) Your judgment will be a part of this process. You may be the one to "start the ball rolling...."

Discussion Questions: (sample question)

2. John Doe has apparently thrown Joe Smith's radio alarm clock across the room, breaking it. You were not in the room at the time but you hurried in when you heard the noise. Joe Smith told you what happened but John is not agreeing with this version of the story. How would this incident be documented in your program? Write or explain how you would write up a report, (make up a possible description of the incident.)

Test also includes matching and multiple choice questions for a total of 15.
